The blood service in question records both sex at birth as well as gender identity: https://www.blood.co.uk/news-and-campaigns/news-and-statements/changes-to-how-we-record-your-sex-assigned-at-birth-and-gender/ The sex at birth is used for the blood processing. The gender identity, as in many other contexts, is recorded for statistical demographics, not for medical uses.
