There's only one recorded instance in 1948 of a soldier apparently poisoning a well "without Haganah General Staff authorisation": https://www.tandfonline.com/doi/full/10.1080/00263206.2022.2122448 Furthermore, the poster alleged that this has happened many more times since. No verified proof is given or can be found online.
