The image is several years old and shows a onesie, not a diaper. A search of the J&J URL at the Internet Archive fails to produce any similar image. A reverse image search leads only to a few conspiracy links with the very same image, including this twitter post from 2019: https://twitter.com/greg_scott84/status/1155694266007470080
