According to the Boston Children's Hospital, you must be 18 years old to qualify for "bottom surgery" and have letters from a three different medical professionals, including your therapist: https://www.childrenshospital.org/programs/center-gender-surgery-program/eligibility-surgery You can get "top surgery" at 15 with two letters from a doctor and your therapist.
