With regard to the first note, the insurance companies death from covid numbers are not reliable. They are taking them from healthcare, where we know medical professionals were encouraged to record deaths 'with covid' as deaths 'from covid' for large bonuses for their trusts. https://www.snopes.com/fact-check/medicare-hospitals-covid-patients/
